- Recognition is ok; including logos.
- No ads except in periodical publications (periodical must
have a date/name/volume #/masthead and be published a minimum 1 per year.
- All money to UW. UW must assume all
liability and receive all benefit.
- No insurance sales, credit card sales, travel
only sales.
- MALINGS DONE IN COOPERATION WITH OTHER AUTHORIZED

- If in Madison MSC (Zips 535, 537, 538, 539) need
only permit # accompanying.
- Mailing outside Madison area must have "Additional
Point of Entry" form submitted to the P.O. where the permit is issued
(Mail Services has these forms available).

When these criteria are not met, mailings can still be sent under permit
658 but the layout of the permit (indicia) stamp must change to delete
the nonprofit status and the mailing must be sent at commercial
rates, which are approximately 80% higher.
Do not violate these regulations. The
USPS penalties for violating these regulations are very severe, including
fines on all non-profit mailings sent by the University of Wisconsin.
